Appointments

To make an appointment, please call 773-631-7898. If you need to cancel your appointment, please call us as soon as possible so that we may use that time to see another patient in need of care. We make a sincere effort to adhere to our schedule and appreciate your patience if we are late due to surgery or an emergency.

Once you have scheduled your appointment, you might be directed to complete various forms prior to your visit. Though not required, filling them out in advance will expedite the registration process. Several of these forms are available here for your convenience.

When making an appointment, please provide the office staff with a current email address to gain access to your patient portal.

Fees & Payments

We do all we can to minimize the cost of your medical care. That is why we require all of our patients to pay for services at the time they are rendered unless prior arrangements have been made. For your convenience, we can accept cash, checks, MasterCard, Visa and Discover Card.

Insurance

We accept most major insurance plans. If we are a participating provider with your insurance company, we will bill them. However, while we can assist with handling claims, payment is ultimately the patient’s responsibility.

We are currently a preferred provider for IPN:
Workers Compensation, Personal Injury, and Accident patients.

Prescriptions

Please request all prescriptions and refill authorizations through the patient portal. These may be requested between the hours of 9:00am–5:00pm, Monday through Friday, when our full records are available. Requests made outside of these hours will be filled only in the event of an emergency or extreme circumstances.
